As part of our Complaints and Feedback Policy we have created this form for Clients, Stakeholders, Friends, Family and Support Workers to report feedback in the best interests of participants funded under the NDIS. By completing the below form this will then be sent to the Complaints Manager who will investigate and report as required.

If participants or stakeholders feel the complaint cannot be resolved internally, they are encouraged to contact below with the Commission.
